Player Registration Form
This form is for players who qualify for scholarships. Please complete the form and mail to the registrar, Jill Cohan 1134 Sherman Ave. Madison 53703

Release of Liability and Consent for Medical Treatment
Each parent must complete and sign these forms. Submit the completed form to the coach of your players team.



Juggling Awards
Kids can get medals, trophy, a or tee-shirt by juggling a soccer ball. See the attached flyer to see the details. There are different levels for the age groups. If a player age 9 or under can keep the ball in the air for 5 touches, they win a medal, 10 touches for U12, and 25 for U14. 50 touches wins a trophy and 100 touches wins a amazing tee-shirt.
Encourage your players to give it a try. Juggling is something they can do on their own and they get a better feel for the ball without even knowing it.

Apply NOW for a Wisconsin Youth Soccer Association (WYSA) Coach's Pass (It's VERY easy!)
The Madison East Soccer Club will pay (see note below) for the fees associated with our coaches obtaining Coach Pass Cards from the Wisconsin Youth Soccer Association (WYSA).
This is a mandatory MAYSA/WYSA requirement that every a coach and manager is registered with the WYSA and has been issued a coach pass!

NOTE - Although the club will pay the $10 fee it would be very helpful if you paid the fee yourself - thanks for considering this option!

TEAM CONTACTS: You must confirm the new date, time and field with MESC Field Coordinator Keith Gilkey (president@madisoneastsoccer.org) and MESC Referee Assignor Jhayson Anderson (n64professor@hotmail.com) so that the game can be re-posted on the Arbiter referee/scheduling system.
